ACCESS to Pharmacy Care enables community pharmacy participation in CMS ACCESS, supporting national scale and sustainable engagement. Participation is open to NCPA members and CPESN participating pharmacies.
CARE TRACKS
Outcome metrics and payment model
CMS ACCESS is the largest service-based program ever available to pharmacists. Payment under the new model will be driven by improved health outcomes for Medicare patients with chronic conditions as described in the four tracks below. It will also test whether technology can improve patient outcomes and lower costs.
