Is ACCESS a good fit for your pharmacy?

This page is intended for pharmacies that are not yet enrolled. In about 15 minutes, pharmacy owners, managers, and clinical leads can understand the ACCESS model, review basic enrollment expectations, and decide whether to move forward with enrollment readiness steps.

Is ACCESS right for your pharmacy?

ACCESS to Pharmacy Care helps pharmacies participate in CMS ACCESS by connecting pharmacy-based chronic care workflows with GamePlan Medical, the CMS ACCESS participant. The program is designed for pharmacies that want to build on existing patient relationships, med sync or appointment-based model workflows, adherence support, care coordination, and follow-up.

ACCESS may be a good fit if your pharmacy can:

  • Identify Original Medicare patients who may qualify for chronic care support.

  • Use med sync or ABM touchpoints to support ongoing care.

  • Assign a clinical lead and complete required training after enrollment.

Participating pharmacies should be ready to:

  • Confirm patient fit before submission.

  • Obtain consent and support eligibility/alignment steps.

  • Collect required baseline measures and document follow-up care.

Before moving forward, confirm:

  • Your med sync readiness or transformation pathway.

  • Your team has capacity to start with a small number of high-feasibility patients.

  • Your pharmacy can follow program documentation and training expectations.

PHARMACY READINESS

Pharmacy requirements to participate

Before enrollment, confirm your pharmacy can meet the core ACCESS readiness expectations below. Pharmacies that do not currently meet the med sync threshold may still have a pathway through qualifying transformation efforts.

Meet med sync threshold of 30% or complete a qualifying transformation effort

Identify a pharmacy clinical lead

Submit required clinical lead name, email, and phone number

Complete required ACCESS training modules after enrollment (~1.5 hours)

Med sync threshold

Follow the steps below to confirm your pharmacy meets the 30% med sync requirement and is ready to enroll in ACCESS to Pharmacy Care

STEP 1

Calculate Your Med Sync Percentage

Use either calculation method to determine whether your pharmacy meets the 30% med sync threshold.

Method one is typically easier. Use the guide for step-by-step calculation support.

Method one: Prescription-Based

Number of prescriptions in sync ÷ total prescriptions over the past 100 days.

Method two: Patient-Based
Number of patients in sync ÷ total patients with fills over the past 100 days.

STEP 2

Use Your Result to Determine the Enrollment Pathway

After calculating your med sync percentage, use this threshold guide to determine whether your pharmacy meets the requirement or needs to complete a qualifying transformation effort before moving forward.

30%+ MEETS REQUIREMENT

Pharmacy has achieved the 30% med sync threshold, and with NCPA membership or CPESN participation may move forward with ACCESS to Pharmacy Care Program enrollment.

If pharmacy does not yet meet the 30% med sync threshold, consider moving forward by completing one qualifying transformation effort:

20–29.9% Needs Improvement

Qualifying Transformation Efforts:

Option A: Achieve 30% med sync.

Option B: Improve med sync by 5 percentage points.

Option C: Enroll in the NCPA CPESN Fellowship and successfully complete attendance and homework for 1 month.

0–19.9% Needs significant Improvement

Qualifying Transformation Efforts:

Option A: Improve med sync by 10 percentage points.

Option B: Enroll in the NCPA CPESN Fellowship and successfully complete attendance and homework for 3 months.

AFTER ENROLLMENT

What happens after your pharmacy enrolls?

Once enrollment is processed, your pharmacy will receive the next steps and the tools needed to begin onboarding, training, and implementation planning.

Visit the Training Center

The Training Center is available to pharmacies after enrollment is completed.

Complete Required Modules

Required training modules are available after enrollment and must be completed prior to receiving patient lists.

Use Implementation Resources

Along with the training modules, implementation resources are available to pharmacies after enrollment.

Ongoing support

Office Hours information will be shared after enrollment so pharmacies have a recurring place to ask questions, clarify workflow steps, and get implementation support. For questions, contact access@ncpa.org.
